How It Works
Follow the steps below...
Step 1
First time customers fill out our Medical Intake Form. You will receive a confirmation email once complete. Returning customers may skip this step unless something has changed since your last visit.
Step 2
Book Your Desired Service. A hold will be placed on your card until services are provided.
Step 3
Our staff will Review your Intake Form and will thereafter contact you via email, phone or text with an official booking confirmation.
Returning Clients
If you are a returning client, you may skip filling out the intake form unless something has changed since your last visit. Just book your appointment!
