How It Works Follow the steps below... Step 1 Book Your Desired Service. A hold will be placed on your card until services are provided. Step 2 Once you book an appointment, the Thank You page will contain a link to our Medical Intake Form. A link will also be provided in your booking confirmation email. Step 3 Our staff will Review your Intake Form and will thereafter contact you via email, phone or text with an official booking confirmation. Returning Clients If you are a returning client, you may skip filling out the intake form unless something has changed since your last visit.
